Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

The state-of-art research in the field of life’s organization confronts the need to investigate a number of interacting components, their properties and conditions of sustainable behaviour within a natural system. In biology, ecology and life sciences, the performance of such stable system is usually related to homeostasis, a property of the system to actively regulate its state within a certain allowable limits. In our previous work, we proposed a deterministic model for systems’ homeostasis. The model was based on dynamical system’s (...) theory and pairwise relationships of competition, amensalism and antagonism taken from theoretical biology and ecology. However, the present paper proposes a different dimension to our previous results based on the same model. In this paper, we introduce the influence of inter-component relationships in a system, wherein the impact is characterized by direction (neutral, positive, or negative) as well as its (absolute) value, or strength. This makes the model stochastic which, in our opinion, is more consistent with real-world elements affected by various random factors. The case study includes two examples from areas of hydrobiology and medicine. The models acquired for these cases enabled us to propose a convincing explanation for corresponding phenomena identified by different types of natural systems. (shrink)

Patient safety has only recently been subjected to wide-spread systematic study. Healthcare differs from other high risk industries in being more diverse and multi-contextual, and less certain and regulated. Also many patient safety problems are low-frequency events associated with many, varied contributing factors. The subject of this paper is the epistemology of patient safety (the science of the method of finding out about patient safety). Patient safety research is considered here on the background of a risk management framework which requires (...) researchers to: • Understand the context - as a subset of healthcare quality, services and systems research, with technical and human behavioural (cultural) components and a range of external and internal organisational influences, a wide range of research disciplines is necessary • Identify the risks - identify the things that go wrong and the frequency and nature of different types of incidents from sources such as medical record review, observational studies, audit, incident and medico-legal reports • Analyse the risks - deconstruct the things that go wrong, identifying contributing factors and trying to detect trends and patterns in contributing factors, detection, mitigation factors, ameliorating factors and actions taken to reduce risk • Evaluate the risks - decide on priorities, identifying preventive and corrective strategies and judging the risk- and cost-benefit of potential corrective strategies such as standardisation or simplification of a process or device • Manage the risk - evaluate and scope preventive and/or corrective strategies and then implement these, or place the problem on a risk register pending solution, or accept that what is needed is unaffordable • Communicate and consult - use interactive sessions, audit, on-going feedback, reminders and patient mediated prompts • Monitor and review the state of the problem - get baseline trends and patterns so that changes can be tracked and properly attributed to an intervention A hierarchy of levels of evidence has been proposed for clinical research and we argue that insufficient weighting has been given to lower ranked levels of research and to qualitative research, although critical interpretive synthesis is now gaining acceptance in mainstream thinking (e.g. by the Cochrane Collaboration). Fundamental challenges remain including how to grasp the elusive concept of patient safety, how to quantify, characterise and cost the problems, how to judge the extent to which harm can be attributed to errors, violations or system failures, how to identify contributing factors and the extent to which they can be implicated, how to judge whether incidents or their precursors are preventable, how to generate strong evidence to make healthcare safer and how to translate research into practice. Future directions include addressing the mundane as well as rare, dramatic events, and developing further research in non-hospital settings and in developing countries. In summary, a mixture of qualitative and quantitative methods, using information from all available data sources and combining retrospective, real time and prospective study designs, is necessary to address some of the more difficult patient safety problems. Some statements couched in the present tense have no reference to time. They are, if you like, grammatically tensed but logically tenseless. Mathematical statements such as ‘twice two is four’ or ‘there is a prime number between 125 and 128’ are of this sort. So is the statement I have just made. To ask in good faith whether there is still the prime number there used to be between 125 and 128 would be to show that one did not understand (...) the use of such statements, and so would any attempt to answer the question. It is tempting to take another step and talk of such timeless statements as statements about timeless entities. If the number 4 neither continues nor ceases to be twice two this is, surely, because the number 4 has no history of any kind, not even the being a day older today than yesterday. Other timeless statements might shake our confidence in this inference: ‘clocks are devices for measuring time’ is a timeless statement, but it is not about a class of timeless clocks. But, given a preoccupation with a favoured set of examples and a stage of thought at which men did not distinguish the properties of statements from the properties of the things they are about, we can expect timeless entities to appear as the natural proxies of timeless statements. In their 1993 article "'Theoretical Mathematics': Toward a Cultural Synthesis of Mathematics and Theoretical Physics" published in the Bulletin of the American Mathematical Society, the eminent mathematical physicists Arthur Jaffe and Frank Quinn proposed a set of prescriptions for the interaction between mathematicians and theoretical physicists that should foster mathematicians' receptivity of ideas from physics by safeguarding mathematical rigour against uncontrolled speculation. The proposal propelled and intensive debate in the Bulletin and lead to a special issue of the journal Synthese. (...) So far, however, philosophers of science have hardly addressed this debate, perhaps because it lacks a systematic framework. In this contribution I portray the various stances taken in the Jaffe-Quinn debate from the perspective of Imre Lakatos philosophy of mathematics that seems most suitable for a debate concerning mathematical growth. After outlining the lessons to be drawn for it from Lakatos's published works, I examine which modifications of Lakatos's account are in place to enable a fruitful assessment of present mathematical physics in which axiomatization plays a key role. Understanding a proposition for an intelligent agent is an important epistemic concept. We first discuss intuitively general logic characteristics of understanding, and give a language and a semantics containing understanding as a modal operator. Secondly, we develop the system LU for the operator, give some results of its proof theory, and then we prove the frame soundness and frame completeness of LU.

The psychological state and neural correlates of 19 women undergoing normal childbirth were studied. Frequent occurence of nonordinary psychological phenomena was traced back in 6 subjects, occurring in active and passive alertness, as well as hypnagogic/hypnopompic periods, and in dreaming, during the late period of pregnancy, giving birth, and the period of 2-4 days post partull. The brain activity of the aforementioned subgroup during this time was characterized primarily by general activation of the right hemisphere in the bandpass of infraslow (...) electrical processes and by more localized EEG activation. The implications of the data for research on gender-specific altered states of consciousness are discussed. (shrink)
